For more coverage you have two options: 
Enrolling in Medicare Part B
Complete the Medicare Part B enrollment application (CMS40B) and submit via fax or in person to your local Social Security office.  If you are retiring you will also need to have your employer fill out form CMS-L654E.   Both applications are below.

Note When completing the CMS L564 Employer Form: 

- State on the form “I want Part B coverage to begin (MM/YY)”
- If possible, your employer should complete Section B. 
- If your employer is unable to complete Section B, please complete that portion on behalf of your employer without your employers signature and submit one of the following forms of secondary evidence:
        - income tax form that shows health insurance premiums paid;
        - W-2s reflecting pre-tax medical contributions;
        - pay stubs that reflect health insurance premium deductions;
        - health insurance cards with a policy effective date;
        - explanations of benefits paid by the GHP or LGHP; or 
        - statements or receipts that reflect payment of health insurance             premiums.

It takes about 2 weeks to receive your medicare cards. SSA will send a confirmation letter in the mail first.

Option 2
A Medicare Advantage Plan
A plan offered by private companies that combines Part A & Part B into one plan.  These plans generally include built in prescription drug coverage and offer additional benefits not provided by Original Medicare.  
Option 1 
These plans often have extra benefits like Dental, Vision allowances, telemedicine, gym memberships and wellness rewards.  
A Medicare Supplement Plan
Often referred to as Medigap plans.  These plans are secondary to Original Medicare and benefits vary depending on the plan type whether A, B, D, G, K, L, M, N, C & F.  Rates and rules also vary by county.  
These plans commonly have lower monthly premiums and cost sharing based on the plan.  
These plans commonly have higher monthly premiums but lower out of pocket on cost sharing. 
These plans will work with any provider or facility nationwide that accepts Medicare.  
Medicare Part D - Stand-a-lone prescription drug plan
The plans help pay for prescription drug costs.  You would need to pair a part d plan with any Medicare Supplement plan.  The two plans work together to fill in the holes of Original Medicare.
